Heart Month Discount App Interest Form

In recognition of American Heart Month, IAC is pleased to offer a special discount to facilities performing services dedicated to improving patient care in cardiovascular health.

New applicants in any IAC modality may qualify to receive a 15% discount off the application fee.

To qualify:

Facilities must submit the interest form below between 2/12/2026 and 3/12/2026 to confirm with IAC your intention of applying for a new accreditation in any modality.*
The discount is offered to facilities applying for IAC accreditation for the first time OR already IAC-accredited facilities that apply in a new modality.
Your facility will have until the end of 2026 to submit the application to receive the discount.

Please note: This is an interest form only, not an official application for accreditation. IAC staff will contact you to confirm details on starting your application for accreditation.
